Welcome to Finesse Health PLLC ("Finesse Health," "we," "us," or "our"). These Terms of Service ("Terms") govern your use of our website, telehealth services, and healthcare services. By accessing our services, you agree to be bound by these Terms.
PLEASE READ THESE TERMS CAREFULLY BEFORE USING OUR SERVICES.
1. Acceptance of Terms
By accessing or using the Finesse Health website, scheduling appointments, or receiving healthcare services, you acknowledge that you have read, understood, and agree to be bound by these Terms and our Privacy Policy.
If you do not agree to these Terms, you may not use our services.

3. Eligibility
To use our services, you must:
- Be at least 18 years of age
- Be a resident of Connecticut (for telehealth services)
- Provide accurate and complete information
- Have the legal capacity to enter into a binding agreement
4. Healthcare Services
4.1 Provider-Patient Relationship
A provider-patient relationship is established only after:
- You complete our patient intake process
- Your provider reviews your medical information
- You receive a consultation with our provider
- Both parties agree to proceed with treatment
4.2 Scope of Services
Finesse Health provides concierge telehealth and in-person healthcare services including:
- Executive wellness assessments
- Hormone optimization (testosterone, thyroid, etc.)
- Medical weight loss (GLP-1 medications)
- Metabolic health optimization
- Preventive medicine consultations
4.3 Limitations
We do not provide:
- Emergency or urgent care services
- Primary care for all conditions
- Services outside of Connecticut
- Services to patients under 18 years of age
- Controlled substances (Schedule II narcotics)
5. Telehealth Consent
By using our telehealth services, you acknowledge and consent to the following:
5.1 Nature of Telehealth
- Telehealth involves the use of electronic communications to enable healthcare providers to deliver services remotely
- We use Doxy.me, a HIPAA-compliant video platform, for telehealth consultations
- Telehealth is not a substitute for in-person care when physical examination is necessary
5.2 Risks of Telehealth
- Technology failures may interrupt or delay services
- Electronic communications may be intercepted despite security measures
- Limitations in physical examination may affect diagnostic accuracy
5.3 Connecticut Telehealth Compliance
Our telehealth services comply with Connecticut Public Act 21-9, including:
- Verbal consent obtained at the start of each telehealth session
- Verification of patient location within Connecticut
- Documentation of telehealth encounters in your medical record
6. Payment Terms
6.1 Fees and Payment
- All fees are disclosed before services are rendered
- Payment is due at the time of service unless otherwise arranged
- We accept major credit cards, HSA/FSA cards, and ACH payments
- Payment processing is handled securely through Paya (PCI DSS compliant)
6.2 Insurance
Finesse Health is a private-pay, concierge practice. We do not:
- Bill insurance companies directly
- Accept Medicare or Medicaid
- Guarantee insurance reimbursement
We can provide superbills upon request for you to submit to your insurance for potential out-of-network reimbursement.
6.3 Membership Programs
Membership fees are:
- Billed monthly in advance
- Non-refundable for the current billing period
- Subject to change with 30 days' notice
7. Cancellation Policy
7.1 Appointment Cancellations
- 24-Hour Notice Required: Please cancel or reschedule at least 24 hours before your appointment
- Late Cancellation Fee: Cancellations with less than 24 hours' notice may incur a $50 fee
- No-Show Fee: Missed appointments without notice may incur the full consultation fee
7.2 Membership Cancellations
- Memberships may be cancelled with 30 days' written notice
- No refunds for partial months
- Outstanding balances must be paid before cancellation is processed
8. Refund Policy
- Services rendered are non-refundable
- Medication costs are non-refundable once shipped
- Lab fees are non-refundable once ordered
- Refund requests for unused services will be considered on a case-by-case basis
9. Intellectual Property
All content on this website, including text, graphics, logos, and images, is the property of Finesse Health PLLC and is protected by copyright and trademark laws. You may not:
- Copy, reproduce, or distribute our content without permission
- Use our trademarks without written authorization
- Create derivative works from our content
10. User Responsibilities
You agree to:
- Provide accurate and complete health information
- Disclose all medications, supplements, and health conditions
- Follow your provider's instructions and treatment plan
- Attend scheduled appointments or cancel with proper notice
- Pay for services as agreed
- Use our services only for lawful purposes
- Not share your account credentials with others
11. Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW:
- Finesse Health shall not be liable for any indirect, incidental, special, consequential, or punitive damages
- Our total liability for any claim shall not exceed the amount you paid for the specific service giving rise to the claim
- We are not liable for any delay or failure in performance resulting from causes beyond our reasonable control
12. Indemnification
You agree to indemnify and hold harmless Finesse Health PLLC, its officers, employees, and agents from any claims, damages, or expenses arising from:
- Your use of our services
- Your violation of these Terms
- Your provision of inaccurate information
13. Dispute Resolution
13.1 Governing Law
These Terms shall be governed by and construed in accordance with the laws of the State of Connecticut, without regard to conflict of law principles.
13.2 Arbitration
Any dispute arising from these Terms or our services shall be resolved through binding arbitration in New Haven County, Connecticut, in accordance with the rules of the American Arbitration Association.
13.3 Class Action Waiver
You agree to resolve disputes on an individual basis and waive any right to participate in a class action lawsuit.
14. Modifications to Terms
We reserve the right to modify these Terms at any time. Changes will be effective when posted on our website. Your continued use of our services after changes are posted constitutes acceptance of the modified Terms.
15. Severability
If any provision of these Terms is found to be unenforceable, the remaining provisions shall continue in full force and effect.
16. Entire Agreement
These Terms, together with our Privacy Policy and HIPAA Notice, constitute the entire agreement between you and Finesse Health regarding your use of our services.
